SUBJECT
Form VE-CE 116 - Completion of Certification Requirement

GUIDELINE DEFINITION
Form VE-CE 116 will be submitted for each certification request indicating completion of a certification educational requirement. This could be for an initial request, when changing part-time staff from Approval status to Part-Time Provisional certification, when renewing Provisional or Part-Time Provisional certification, and when moving from Provisional or Part-Time Provisional to a Five-Year certificate. Multiple university courses can be recognized towards meeting an educational requirement.  Completion of fractional parts of a district in-service activity may be reported, without actual completion of the in-service activity.

Forms produced from local electronic files that can reasonably substitute (as determined by Wisconsin Technical College System Board staff) for a printed VE-CE 116 form will be accepted. The form will not be processed unless signed by the District Certification Officer. Forms produced from local electronic files can utilize a printed signature. If the initial request results in an individual having all of the required educational requirements listed as outstanding requirements, a Form VE-CE 116 does not have to be submitted.

RATIONALE
District Certification Officers are responsible for making decisions regarding completion of educational requirements 50-55, 58-60, and 69-74. The form is utilized to communicate those decisions.
